Lots of people hark on about how rebreather can fail easily and kill you instantly. Here I've tried to list ALL the possible failure modes and how you can get out of them. All this is covered in your training and will become second nature. In fact ALL failures are possible to easily escape from providing you monitor the unit and do your pre dive checks
I am also only covering problems unique to the Inspiration as we still have all the normal diving problems to cope with but these are covered on lots of other sites. Please let me know if I've missed anything
There does seem to be a lot of things that can go wrong at first glance, but most of these will never happen if you do your pre-dive checks. Your training will teach you how to prevent and deal with these. If it didn't then please let me know

Tut, tut, Who forgot to fill that cylinder! Or diving with not enough O2 for the dive. Maybe a leak somewhere, but you notice bubbles on a rebreather

ALWAYS analyse your gasses after a fill. You'd be amazed how many places don't give a 100% fill, or nowadays, how many air fills aren't
On these two fills one is 96% and the other 99.8%. I always analyse and label my gases myself. Never trust a shop
