How to Apply for a Child's Performance Licence

How to Apply for a Licence

A licence is issued by the area of the LEA in which the child lives (this may not necessarily be the same Authority as the child's school, nor the intended place of performance).

The application should be made on the LEA application form.  

  • The applicant completes Part 1

  • The parent/carer of the child named in the application completes Part 2

  • The school completes Part 3 (or a letter from the school would suffice). 

The completed application form must be accompanied by the following documents: 

  1. a copy of the birth certificate of the child (or other satisfactory evidence of the child's age);

  2. two identical prints (unmounted) of a photograph of the child taken during the six months preceding the date of the application;

  3. a copy of the contract, draft contract or other documents containing particulars of the agreement regulating to the child's appearance in the performance or regulating the activity for which the licence is requested;

  4. if the child is under 14 years of age - a declaration under section 38(1 )(a) or (b) of the Children and Young Persons Act 1963 (the 1963 Act);

  5. medical certificate if required (see Medical Requirements).

The licence will show the following information: . 

  • name, address and date of birth of child;

  • photograph of the child;

  • identity of the licence holder; 

  • name and nature of performance; 

  • part in performance; 

  • place of performance; 

  • date(s) of performance;

  • performance start/finish time;

  • anticipated arrival/departure time; . rehearsal date(s);

  • date(s) absent from school (if applicable); . name of Chaperone/tutor; . other LEA area(s);

  • any conditions stipulated by the LEA, these could include for example the permitted hours, transport arrangements and educational provision;

A licence will not be issued unless the LEA is satisfied that the child is fit to take part in a performance, that his/her education will not suffer and that proper supervisory arrangements have been made to secure the child's health, comfort and well-being.

If the child is not a resident of the UK, application should be made to the LEA in whose area the applicant resides, or has his place of business.
