The following forms can be downloaded for your use when applying for a licence or Chaperone status.

Yellow Form

Register of children taking part in a performance not requiring a Licence

ONLY complete this form if the child takes part in a performance which totals four days or less, is not being paid, is not seeking school absence and is not being broadcast.
However, it is important that this form IS USED where appropriate, as WCC will need to monitor the number of performances each child undertakes.

NOTE - it will only actually be yellow if YOU print onto yellow paper!!

Application for licence for use of children in performance

To be completed and submitted by the producer (or designated child performance co-ordinator) of the production company

Forms can also be requested at the theatre - see Tony Deeming.

CRB forms are individually reference numbered and as such are not available as downloads, but can be requested at the theatre. However, should you require a continuation form then you can download a pdf copy by clicking here.
