PATHOLOGY – THE ESSENTIAL SERVICE : DRAFT GUIDANCE ON MODERNISING PATHOLOGY SERVICES
A CONSULTATION DOCUMENT FROM THE DEPARTMENT OF HEALTH
In June 2002, the Department of Health issued this consultation document which sets out the Department’s proposals for modernising NHS pathology services and presents draft guidance for consultation.
Pathology services cannot stand still. The NHS is changing, patients' expectations are higher, and staff want to work in new ways and develop innovative services for patients. Patients and staff need faster access to diagnostics in primary care, A&E, out-patient clinics and on the wards. We know that pathology services can also be provided in new ways and new settings, through point of care testing in GP surgeries and community clinics. Many patients with long-term conditions are already testing and managing their condition in the community and more can do so. A modernised NHS is only possible with modernised pathology services.
The NHS is changing. The traditional image of the DGH as the principal provider of healthcare is no longer accurate. Individual trust-based pathology services can be poorly placed to deliver patient choice, improved access to testing in new and different settings, reduced isolation for pathology professionals, increased specialisation, and the demands of clinical governance and accreditation. While informal pathology networks are common across the NHS, experience has shown that they seldom go far enough to deliver change fast enough and they struggle to make their management arrangements clear enough for effective clinical governance.
Based on growing experience in the NHS, we believe that integrated networks offer the best way to plan and manage a comprehensive pathology service to meet the needs of local health systems, while their larger size allows new technologies and modern information systems to be brought in, leading to increased quality and efficiency. They allow more efficient deployment of staff and equipment and more effective sub-specialisation. They also offer better opportunities for training, education and career development and provide increased professional support. The key feature of a managed pathology network is a single, integrated management structure and budget.
To be effective, we would expect networks to serve populations equivalent to those served by StHAs. When developing pathology services, health communities will need to consider the benefits of networks mirroring StHA boundaries, and the natural patient flows from primary to secondary to tertiary care when determining network boundaries.
The concept of networks is not synonymous with centralisation. Where networks have been set up, appropriate services have been maintained on individual hospital sites. However, networks can also deliver benefits in quality and efficiency by consolidating some aspects of the service into a smaller number of sites.
Working together in new ways and across traditional boundaries, providing opportunities and support for flexible working, developing medical and scientific understanding, applying them in new diagnostic techniques - all these are vital if we are to give patients what they need - high quality, comprehensive, efficiently managed diagnostic services.
These changes can benefit the pathology workforce too. The NHS needs to attract and hold on to staff with the right skills and expertise. We can do this by offering them interesting work, professional challenges, high quality education and training, opportunities for research and development and an integrated career structure. We can also offer them a good quality working environment, with more effective supervision arrangements and better development opportunities. We need their input to pathology modernisation to improve services for patients. This is a positive opportunity for all members of the pathology community to become involved in shaping the future of their working environment and practice for the better.
