We can design forms like the above to work simply to collect information and allow it to be sent by e-mail. We can also extend the functionality of such a  form  to allow it to be executed as a standalone desktop application rather than through a browser.

In addition we can design and implement applications of this type (with form front ends) which interact with spreadsheets, databases in addition to e-mail applications.

We also have the toolsets to allow design and implementation of Microsoft .net form based applications using Visual Basic .net or other languages and again connecting to databases either on a local LAN or on a web server.
