Car parking is available at the
surgery. There is wheelchair access to the surgery, consulting
rooms and toilets. Prams may be left in our enclosed porch.

COMMUNITY AND SOCIAL SERVICES
We have a unique arrangement whereby
the following services may be obtained through the Surgery –
District Nursing, Health Visiting, Social Services, Psychiatric
Nursing, Midwives, Dietician, Podiatry (Chiropody), Physiotherapy,
Occupational Therapy, Home Help Services and Care of the Elderly.

ROUTINE TELEPHONE CALLS
The Doctors can be accessed
by telephone and the best times to call are between 8.30am and
8.55am or 10.30am and 11.00am each morning. If it is difficult
for you to telephone at these times due to work commitments etc,
please ring when able and we will try to accommodate you.

EMERGENCY CALLS:
During the day from 8.30am telephone 028
9061 8211 and our staff will contact the doctor. After 6.00pm,
at weekends and Bank Holidays please telephone the Lisburn Out
of Hours Service on 028 9260 2204 where a receptionist will take
the details.

HOME VISITS
We are willing to visit you if you are unable
to come to the surgery because of illness. PLEASE TELEPHONE REQUESTS
BEFORE 10.30am so that we can plan our day. You will be asked
to give the name and address of the patient as well as symptoms
to help us assess the urgency of the visit.

MISSION STATEMENT
"In the running of this Practice
the clinical care of the patient will be of paramount importance.
Our care is based on the patient being seen by the Doctor they
know best and who knows them. Proven medical advances will be
blended with traditional care. The Practice aims to offer a quality
service through the provision and development of primary healthcare,
maximising all available resources to the optimum benefit of patient
health.

PATIENT PARTICIPATION
GROUP
In February 1996 a Patient Participation
Group was formed in conjunction with Community Social Services.
Patients from a variety of backgrounds were invited to become
members eg patients who are retired, patients with young children,
patients with a disabled family member, patients who have contact
with the elderly in the community.
We meet three times a year and publish Newssheets which are available
in our Waiting Room and are taken to our housebound patients by
the District Nurses.
Any patients who are interested
in joining the Group please contact the Practice Manager.

REPEAT PRESCRIPTIONS
In the interest of good medical care the
doctors like to see patients who require medication for long term
illness. These patients should make an appointment each time they
need to renew their prescription.
Other repeat prescriptions may be requested in person, by telephone
or by letter. If you wish to have your prescription posted, please
enclose an S.A.E.
Please give us at least 24 hours notice and do not wait until
you have no medication left

TIPS TO MAKE BEST
USE OF THE PRACTICE
If you have many problems to discuss
with the doctor, it is helpful if you make a double appointment.
Please let us know if you change
your address or telephone number.
If you have to see the doctor for
your prescription, please make an appointment about a week in
advance then you are more likely to get a time that suits you.
Order your repeat prescriptions
by phone before 11.30am and it will be ready the same day at 1.00pm.
If ordered after 11.30am, the prescription will be ready at 1.00pm
the following day.
Always have to hand the name and
the strength of the drug when you are ordering by phone. The receptionist
(and sometimes even the doctors) do not know what "the wee
white pill for my heart" is. There is a counterfoil listing
your drugs, attached to your prescription, which can be used to
order from.

SUGGESTIONS/COMPLAINTS
We are always happy to consider
any suggestions you have with regard to improving our service.
Please write down any suggestions and these can be left at reception
or in the post box on the railings outside the front door of the
surgery.
We would be glad to receive your
input.
Complaints, whether formal or informal,
should be addressed to the Practice Manager or any Doctor.
We are also grateful for compliments
as these tell us we are doing a good job and gives us the encouragement
to keep improving our service.

WHEN PATIENT INFORMATION
MAY BE USED
As well as being used for
clinical care and treatment, your records may also be used for
clinical audit, teaching, statistical analysis and verifying payments
made to your doctor.
We only ever use or pass on information
about you if people have a genuine need for it. Whenever we can
we shall remove details which identify you. The sharing of some
types of very sensitive personal information is strictly controlled
by law.
Anyone who receives information
from us is under a legal duty to keep it confidential.
You have a right to object to the
use of your information for any of these purposes.

HOW DO I REGISTER WITH THE PRACTICE?
If you live within a five mile
radius of The Hill Medical Group, or a family member is already
registered with us, you can register with the Practice by simply
calling in person to the surgery or telephoning (028 90618211)
for an appointment with your chosen Doctor. You will be asked
to bring your Medical Card with you to your appointment. If you
do not have your Medical Card you will be given a form to complete.
The appointment will be a chance for you and the doctor to meet.
You will be given a new patient
medical at the appointment. A new patient medical consists of
a short medical history, blood pressure, height and weight check
and urine test.
