This form outlines the purposes of the study and provides a description of your involvement and rights as a participant.

You are encouraged to ask any questions at any time about the nature of the study and the methods that I am using.  Your suggestions and concerns are important to me; please contact me at any time at the email address listed above.

I guarantee that the following conditions will be met:

1)  No data will ever be presented with your name on it, unless you specifically give permission. Every attempt will be made to ensure complete anonymity of the participants at all times. Wherever possible, entries will also be de-contextualised. 

2) Your involvement in this research is voluntary. Your participation will not influence in any way the evaluation of your performance on the course. You may change you mind about participating in the research enquiry and withdraw your data from the study at any time.

3) All data and results will be subject to strict confidentiality.
